Service Description

To safeguard the health of low-income women, infants, and children up to age 5 who are at nutritional risk by providing nutritious foods to supplement diets, information on healthy eating, and referrals to health care. Participants receive a nutrition assessment, health screening, medical history, body measurements (weight and height), hemoglobin check, nutrition education, breastfeeding support, referrals to other health and social services and vouchers for healthy foods.

Residence (proof of home address, must live in state in which you are applying Income (proof of total family, gross income must be at or below 185% of poverty) Category (pregnant, breastfeeding, non-breastfeeding, infant, or child under 5 years) At nutritional risk (determined at certification)

Application Instructions

Call local WIC office. Clients must show proof of income, home address, and identification at the first visit.
