Application Instructions
Access website www.uscis.gov, click on the office locator link, then click on "Find a doctor for an immigration medical examination", which is found at the bottom of the page. Enter your zip code in the locator field for a list of Civil Surgeons in your area. When making the appointment with the civil surgeon, please indicate that you require a medical examination and inquire about documents you need to bring.
Call the automated USCIS Information and Customer Service number at 1-800-375-5283, choose option 5, follow the prompts and you receive information about a Civil Surgeon in your area. Hearing impaired individuals may call 1-800-767-1833.
