Swag Family Resource Center

Provides medical and dental services, including, Sexually Transmitted Infection (STI) testing and treatment, family planning, STD testing and treatment, Human Immunodeficiency Virus (HIV) testing, adult physicals, clinical care, extractions, fillings, cleanings, and comprehensive exams.

Individuals ages 18 and over.

Call to make an appointment or to ask if a walk-in spot is available. For Dental exams, walk-ins are available once per morning once per afternoon. Walk-ins will be first-come-first-served and for emergencies only.

Required Documents: Income verification Picture ID or other forms of ID such as voter registration card, birth certificate, hospital records, pay stub, etc. will be accepted

Medicaid and Medicare, offers sliding scale fee; call for details.
