Documents required for online upload: A Parent or Guardian ID that shows Broward County residency. Child's birth certificate with parent's name, footprints (from a hospital for children under a year), or Vaccination Records showing parent's and child's name. Documentation showing approved legal guardianship if an individual has legal custody of the child. Proof of income (Paystubs, Unemployment Letter, Food Stamps Letter, etc.)
