Service Description
A nursing home diversion program that provides in home and community services to individuals over the age of 65 that have Medicare Parts A and B, and are eligible for Medicaid. The program provides case management and services, such as personal care, homemaking, assisted living, adult day care and medical supplies. In addition to meeting age criteria, individuals also must meet income restrictions (income less than $2,022 per month) and have assets under $2000.
