Service Description

Provides a comprehensive geriatric care center with a nursing home, ALF's (Adult Living Facility), hospital and facilities to provide at-home and outpatient services. Services include Alzheimer's care and respite, adult day care, caregiver training, mental health and medical care, case management and community outreach. Will evaluate an older person with a team which includes geriatric physician, dietician, social worker, physical therapist and other support persons. Strives to keep individuals living independently as long as possible.
