Service Description
Provides primary care services for prenatal and perinatal women at risk during pregnancy or birth in the home or directly at the Primary Care Center. Services include, but are not limited to, childbirth education, breastfeeding services, parenting education, care coordination, midwife assistance, nutritional education, and linkage to WIC and Medicaid.
