Provides psychiatric evaluation and medication management with ongoing follow-up and monitoring. Memorial Outpatient Behavioral Health's psychiatric medication clinic is staffed by board certified psychiatrists and advanced registered nurse practitioners who practice with a holistic, integrated approach.

Eligibility

Individuals ages 18 and over. Medicare patients need referral from a treating physician.

Application Instructions

All psychiatric medication clinic patients must first complete an intake assessment with a licensed mental health clinician. Walk in for same day access for an intake assessment, no appointment needed. Walk in any time, Monday through Thursday between the hours of 8am and 4pm to complete the intake; call for details.

Required documents: Health insurance card (if applicable) Photo ID Proof of residency (if applying for grant funding)

Fees

Most insurance accepted as well as Medicaid and Medicare. Grant funding available for uninsured persons who meet income eligibility criteria.
