Service Description
The Emergency Department Care Coordination Team (ED-CCT) was designed to provide clients ages 0-25 with assistance in transitioning from a Crisis Stabilization Unit to community based care. The team will work with patients on linking them to the appropriate community based services in an effort to enhance their natural supports within the community. In doing so, risk factors resulting in reoccurring visits to the Emergency Department can be reduced
