Provides housing and supportive services to homeless individuals with serious and persistent mental illness. Eligible applicants may receive Section 8 Housing assistance.

Eligibility
Individuals over 18 years old who have existing DSM-V mental health diagnoses meet the Federal definition of homeless. This project gives preference to the chronically homeless.
Application Instructions
Needs a referral form completed and must provide documentation of severe mental illness and homelessness. Preference is given to the chronically homeless. Must call Homeless Helpline at (954) 563-4547.
