Provides a home visit program with one-on-one support throughout a woman’s pregnancy, after the baby is born and during early childhood. Services include education and support in childbirth, newborn care, breastfeeding, child development, nutrition, family planning, smoking cessation, infant safety and more.re.

Eligibility

Pregnant women, infants and families with children up to age 3, at-risk for poor birth outcomes and need help with healthy child development.

Application Instructions

A healthcare provider, nurse or midwife offers the Healthy Start confidential screening, which is a set of questions to see if mom and baby could benefit from Healthy Start services or call a local Healthy Start Care Coordinator to enroll into the program.
