Service Description

Provides trauma services to individuals with mental health, substance use, co-occurring disorders, and psychiatric and medication management needs. The program also utilizes comprehensive assessment, evaluation, and treatment planning processes to determine and address each client's needs, strengths, abilities, clinical severity, and diagnosis and treatment course. Outpatient services focus on helping the client and family learn more effective coping skills for mental health, substance use, or co-occurring disorder and maximizing the client's potential for successful treatment through individual, family, or group therapy.

Eligibility

Individuals at least 3 years old;

Meet the criteria for an Axis I mental health or substance use diagnosis as defined in the DSM 5 or ICD 10: Have completed a more intensive level of care/treatment and require services until recovery efforts result in additional stability, free from intoxication or withdrawal symptoms. Is assessed as not posing a risk of harm to self or others and is not vulnerable to victimization by another. Mental status does not inhibit their capacity and ability to understand the materials presented and to participate in the program. Is medically stable, or are medical issues being concurrently addressed and will not interfere at this level of care/treatment. Has the willingness to cooperate with treatment as it relates to attending all scheduled sessions, adhering to the treatment plan. Demonstrating motivation to obtain a support system.
