Service Description

Offers an approach to pediatric health care whereby children with medically complex needs receive their medical and non-medical support services in a comprehensive, coordinated, and compassionate fashion. Children and families are assigned a medical social worker; other services provided include nutrition, health education, and developmental assessments.

Eligibility
