Service Description
Provides home visitation programs, support, and wraparound services in Broward County that target the family's individualized needs.
Eligibility
All pregnant and new Moms are eligible.
Application Instructions
Referrals are received by pregnant and new moms (self-referrals) or through the OB/GYN, clinics, hospitals, community agencies, or health plans.
Required Documents: Medicaid card
