Provides an application for a U.S. passport. Once processed, the application (along with all required documentation) is sent to the U.S. Department of State, Bureau of Consular Affairs for approval and processing. That takes about four to six weeks if electing standard delivery. However, individuals can pay for expedited services to reduce the processing turn-around time to approximately two to three weeks. If traveling within the next 14 days, they can make an appointment with the Miami-Dade Regional Passport Agency.

First-time passport applicants requires a completed DS-11 form which can be found on the website www.state.gov/documents/organization/212239.pdf or available at the Passport Acceptance facility (must arrive before 2:15 PM). visit the website travel.state.gov for details.

Required documents for frist-time passport applicants: Proof of U.S. Citzenship (original documentation) Proof of ID 2x2 Passport photograph

Required documents for non-first-time passport applicants: Certified birth certificate issued from Vital Statistics (birth cards or hospital issued certificates are not acceptable as primary evidence) Birth certificates with a filing date later than one year of birth or delayed certificates, please call the U.S. Dept. of State for additional requirements Original naturalization certificate Original certificate of citizenship Previous undamged U.S. passport (book or card)
