Service Description

Provides a minimum of seventy (74) units of permanent supportive housing to chronically homeless individuals who have been diagnosed with severe and persistent mental illness (SPMI). Also, may have co-occurring substance use disorders, chronic physical illness, or disabilities that impede their ability to live independently.

Eligibility

Individuals who are chronically homeless and have been diagnosed with severe and persistent mental illness may have co-occurring substance abuse disorders, chronic physical illness, or disabilities that impede their ability to live independently.

Application Instructions

Potential applicants are identified by the Broward County Continuum of Care Coordinated Assessment process. Once identified, Henderson's HOPE team screens the potential applicant for eligibility before referring to BCHA. Please contact Homeless Helpline (954) 563- 4357 or the nearest Homeless Assistance Center to complete required evaluations which drive the Coordinated Assessment Process.
