Service Description
Provides a comprehensive care model to treat persons with opioid use disorders that features phased treatment: Assessment and medical services, intensive outpatient stabilization with integrated primary care services, and maintenance phase with ongoing peer support, medication management, and a compendium of supportive services.

Persons with substance use disorders
Application Instructions
Complete assessment and physical with medical clearance; call for details. May be self referred.
Fees
Accepts Medicaid, and other insurances; offers sliding scale; call for details.
