Service Description

Restores an individual's capacity to operate in the community and their overall well-being after they have been diagnosed with a mental or emotional disorder and may be seen as a psychiatric disability. Guarantees each client's "continuity of care" by service planning, monitoring, advocacy, linkage, and referral.
