Individuals with a documented diagnoses of Cerebral Palsy, Down Syndrome, Spina bifida cystica or myelomeningocele, forms of Autism, Prader-Willi Syndrome, Phelan-McDermid syndrome, Intellectual Disability (IQ below 70), and Individuals between the ages of 3 - 5 at high risk for a developmental disability and their families.

Submit an application to one of the local APD offices by mail or in person to apply for services. Individuals can find locations on the website www.apdcares.org/region along with the application www.apdcares.org/customers/application .
