Service Description

Provides comprehensive information and referral to VA-funded services and community-based services to connect military members, veterans, and their families to help. Also, it offers peer (veterans supporting veterans) and crisis support, such as suicide care supportive services. To provide comprehensive services, Peer Specialists conduct care coordination services, a telephone-based follow-up program to ensure veterans get connected to services.
