Service Description

Provides intensive case management services to help families in the special needs system navigate the care continuum by referring, linking, and advocating for clients. 2-1-1 Broward, in partnership with Jewish Adoption and Foster Care Options (JAFCO).

Eligibility

Individuals 22 years old or younger with Autism, Spina Bifida, Intellectual Disability, Cerebral Palsy, Down Syndrome, Deaf or Hard of Hearing, suffer Visual Impairment, Speech, Language Disorder or, Traumatic Brain Injury.

Application Instructions

Complete a pre-screening with a 2-1-1 Special Needs Specialist.
